New Patients (60-Minute Appointments)

  • All new patient appointments require confirmation by our office.
  • After scheduling online, please expect a phone call from our staff to review what to expect and answer any questions.
  • If we are unable to reach you and do not receive a response within one week, your appointment may be canceled so that the time can be made available to other patients.
  • Our fees are posted under the new patient tab, and they are subject to change year to year.
  • Please plan to arrive at your appointment early to complete forms. We will also electronically send you the forms a few days before your appointment.
  • We require a 2 business day notice to cancel without a late cancellation fee. Both late cancellations and no-shows will incur a $100 fee in order to reschedule.

Follow Up for established patients (30-Minute Appointments)

  • Please review the scheduler – you can select an in person visit or a telehealth visit.
  • If your last visit was more than 3 years ago, please schedule as a new-patient so you can re-establish care and complete new forms.

Services That Cannot Be Self-Scheduled

At this time, the following services must be scheduled directly through our office:

  • Vitamin injections
  • Blood draws / laboratory appointments
  • Breast ultrasound appointments
  • Pap smears

Please call our office, and our staff will be happy to assist you.

After-Hours & Weekend Appointments

We understand that weekday appointments are not always possible and urgent matters may arise on the weekends. Limited evening and weekend appointments are available on a case-by-case basis via telehealth.

These appointments cannot be scheduled online. Please call our office to request an after-hours or weekend appointment. Our staff will confirm the doctor’s availability before scheduling.

Please note there is a $200 surcharge fee for an evening or weekend appointment.

Your first appointment with Dr. Tais will include a comprehensive medical history and in-depth discussion about all your health concerns. We want it to be the most enlightening appointment you've ever had.
